• Provides physician ordered wound care/treatment to all residents.

• Documents information pertinent to the wounds: size, depth, observations, response to treatment and description of wound.

• Communicates and updates staff and the Interdisciplinary Team, as required.

• May be required to hold weekly wound assessment meeting.

Requirements

• term care facility. Must hold a current license in the State of Ohio as an LPN or RN.
