Position Summary Foundation Health is seeking a highly skilled and compassionate Nurse Practitioner to join our interdisciplinary team. The exciting and entrepreneurial role focuses clinic-based and virtual primary care in an innovate and growing healthcare group.

Key Responsibilities

will provide direct patient care as a Nurse Practitioner while supporting the oversight, coordination, and quality management of Foundation Health’s nursing programs.

Part-time position.

Hybrid position - combination of remote and on-site.

Requirements

Candidates must possess current registration with the College of Nurses of Ontario as a Nurse Practitioner (Registered Nurse – Extended Class) and have completed an approved Master of Nursing – Nurse Practitioner program or equivalent. Demonstrated experience in primary care within multidisciplinary teams is required, with leadership or supervisory experience considered a strong asset.
