About the Role

  • The Clinical Pharmacist is responsible for medication order review, pharmacokinetic monitoring, drug information provision, patient education, and participation in clinical initiatives.
  • Review and verify medication orders for appropriateness, safety, and efficacy, considering patient-specific factors such as age, weight, renal function, and allergies.
  • Monitor serum drug concentrations and adjust dosages as needed to achieve therapeutic goals and minimize toxicity.

Requirements

  • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree required.
  • Active State Pharmacist License in good standing in the state in which the employee works (RD PHARM) required.
  • Fire Safety card within 90 days of hire required.
