Key Responsibilities

• Provide virtual high

Requirements

• Obtains credentialing and additional clinical state licenses, as requested by Hazel’s licensing and credentialing departments.
• Maintains a minimum required weekly availability and productivity levels established by Clinical Leadership.
• Required: Licensed without restrictions as a LCSWC, LCMFT, LCMHC
• Required: Licensed in RI and the state in which you reside
• Additional licenses in AL, CA, CO, FL, GA, HI, IA, IL, MA, MO, NC, NV, OH, PA, TX, VA, and/or WA desirable
• Minimum of 1 year experience in a school
