Key Responsibilities

• will you be doing: The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through Croí Health.

• will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care.

Requirements

• It is unlawful in Massachusetts to require or administer a lie detector test as a condition of employment or continued employment.

• An employer who violates this law shall be subject to criminal penalties and civil liability.

Benefits & Compensation

• Salary: $38 - $52/hr

Work Schedule

• Full-Time position.

• Call responsibilities required.

• Weekend coverage may be required.
