Key Responsibilities

will you be doing: The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through Croí Health.

Requirements

Benefits & Compensation

Salary: $38 - $52/hr

Work Schedule

Full-Time position.

Call responsibilities required.

Weekend coverage may be required.
