Key Responsibilities

  • Maintains up to date knowledge of Medicare/Medicaid/JCAHO regulations.
  • Provide psychosocial assessments of patients and family to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.
  • Formulate, develop, and implement a comprehensive psychosocial treatment plan utilizing appropriate social work treatment and interventions.
  • Interventions may include crisis intervention brief and long-term individual, family therapy as well as grief and bereavement work.

Requirements

  • Education and License Work requires a minimum of a Bachelors in Social Work from CSWE accredited school of social work.
  • Masters of social work from an accredited school of social work, preferred.
  • Work Experience 2years of social work experience, in a health care setting.
  • Computer skills required with Microsoft Word and Excel experience preferred.

Work Schedule

  • Full-Time position.
  • Call responsibilities required.
