About the Role
The Clinical Pharmacy Specialist (CPS) works independently within Patient Aligned Care Teams (PACT) in primary care clinics in Alaska, providing advanced medication management.
Key Responsibilities
- Initiate, modify, or discontinue medication therapy.
- Consult on intensive medication therapy management.
- Collaborate with healthcare teams to optimize patient outcomes.
Requirements
- Licensed Pharmacist.
- Ability to work independently under scope of practice.
- Drug test required.
