Key Responsibilities

Provide evidence-based primary care to 10-15 patients daily, focusing on prevention, chronic, and acute care needs. Partner with a care team to develop biopsychosocial patient care plans and provide clinical leadership within the clinic.

Licensure & Certifications

Requires NP Board Certification in primary, family, or internal medicine with a valid ARNP license and DEA number. Candidates must have 3-5 years of primary care experience and 2+ years in case management focusing on population health.
