This position qualifies for federal student loan forgiveness. After 10 years of qualifying payments while working full-time for a nonprofit employer, the remaining balance on your Direct Loans may be forgiven.

Key Responsibilities

  • You will coordinate care between your patient’s community medical providers, and other disciplines to provide updates, obtain new orders. This coordination takes place during and in between visits.
  • You’ll be part of an interdisciplinary team including other providers, nurses, and a medical social worker.
  • We see patients to help with symptom management for serious chronic illness, looking beyond just their diagnosis to help them live life to the fullest.

Requirements

  • Since we are not an emergency service, for urgent or emergent situations, patients should be directed to their Primary Care Provider. Clinic Logistics:
  • On an average day, you will see 5 patients – 2 new patients, and 3 follow up patients. Some of these patients may be telehealth visits.
  • New patient visits typically last 1 hour, and follow
  • up visits are based on provider discretion, usually 30 minutes to 1 hour. What you’ll be doing:
