Join Trinity Hospice as a Hospice Social Worker – On-Call Rotation in United States. Job Summary: The Social Worker will provide social services under the direction of a Physician, to offer support to our patients and their families. Social work services will be based on the patient’s psychosocial assessment, and the patient and family’s needs.
