Develops and coordinates holistic care management activities for members with significant mental and behavioral health needs. Acts as a liaison and advocate while performing regular site visits to assess needs and facilitate community resources.
Requires a Master's degree in Mental Health or Social Work or a degree from an accredited School of Nursing. Candidates must have 2-4 years of related experience and hold a valid professional license such as LMSW, LCSW, LMFT, LMHC, LPC, or RN.
