Description Halcyon Home is hiring a PRN Home Health Occupational Therapist to support our growing geriatric home health population in Hunt County, TX and surrounding areas. We offer excellent per visit rates, and wonderful team support.

Key Responsibilities

• Perform ongoing clinical assessments to determine patient needs.

• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.

• Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.

• Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.

• Promote the Agency’s philosophy and policies to ensure high-quality care.

• Environmental and Working Conditions: Work in patients’ homes under various conditions, with possible exposure to blood, bodily fluids, and infectious diseases.

• Ability to work a flexible schedule and travel locally, with occasional exposure to unpleasant weather.
