Candidates must be a graduate of an accredited Occupational Therapy Program with an Internship or rotation in the appropriate clinical setting, and possess an Occupational Therapist license in Arizona upon hire. A Basic Life Support - CPR certification from the American Heart Association is also required upon hire.
