Key Responsibilities

Perform wound assessments, develop treatment plans, and provide ongoing wound management.

Conduct patient visits in private residences and assisted living facilities within the Livingston, NJ region.

Document clinical findings, treatment plans, and progress notes accurately and in a timely manner.

Collaborate with caregivers, facility staff, and interdisciplinary teams to support optimal patient outcomes.

Requirements

Licensed Nurse Practitioner (NP) authorized to practice in the state of New Jersey.

Experience in wound care strongly preferred.

Strong clinical assessment and decision‑making skills.

Excellent communication, professionalism, and patient‑centered approach.
