Key Responsibilities

• monthly clinical supervision with the Clinical Manager. · Engage in team meetings and case conferences as needed. · Provide Field Instruction to MSW students, when appropriate.

Requirements

• Communicate clinical information in a succinct and comprehensive way to integrated care teams, inclusive of PCP, NP/MD, and Health Home Care Management staff.

Benefits & Compensation

• Salary: $90K - $100K

Work Schedule

• Full-Time position.

• Hybrid position - combination of remote and on-site.
