Key Responsibilities

Participates in IDT conferences observing, assessing and bringing information regarding psychosocial, emotional, spiritual, physical and financial conditions affecting the patient and family.

Documents all patient/family visits, telephone contacts and referral actions timely, accurately and concisely in the patient record.

Conducts reassessments of patient/family needs and counseling as required.

Requirements

Candidates must possess an MSW degree from a CSWE-accredited school of social work and a valid Driver's License with reliable transportation and car insurance. One year of experience in Home Health Care and hospice experience are preferred qualifications for this role.
