Key Responsibilities

Completes assessments for the mental health and PRP programs to identify the needs of participating consumers

Lead and coordinate clinical meetings with providers, therapist, referral partners, etc.

Assist, as needed, in the development of the treatment plans and/or Individual Rehabilitation Plans for each consumer, with short and long

term goals and interventions

Requirements
