Key Responsibilities

  • Completes assessments for the mental health and PRP programs to identify the needs of participating consumers
  • Lead and coordinate clinical meetings with providers, therapist, referral partners, etc.
  • Assist, as needed, in the development of the treatment plans and/or Individual Rehabilitation Plans for each consumer, with short and long
  • term goals and interventions
