Key Responsibilities

at the discretion of the Medical Director.

Requirements

Licensure (or eligibility for licensure) for medical practice in both KY and IN, including DEA and controlled substance permits, required.

Fellowship in hospice and palliative medicine required.

Hospice, palliative care, primary care, hospitalist, or geriatric experience preferred.

Board certification in Hospice and Palliative Medicine or Board-eligible with expectation to be certified within 5 years of employment required.

Work Schedule

Full-time position.

Call responsibilities required.
