-, AND RESPONSIBILITIES: · Review prescriptions issued by prescribers to assure accuracy and determine formulas or ingredients needed.
- · Evaluate prescriber medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability.
- Make recommendations for medication therapy changes to prescribers as appropriate.
- · Collaborate with the healthcare team to provide clinical and technical support as needed.
Requirements
- · Graduate of ACPE accredited School of Pharmacy.
- · Current licensure in the state of practice.
- · Long-term care pharmacy experience preferred.
- · Experience in patient care and medication management, drug therapy monitoring, drug and medical supply procurement, inventory control management, and performance improvement.
Work Schedule
- Part-Time position.
