Key Responsibilities

Key responsibilities include: Support consumers in achieving personal care goals and addressing ongoing challenges.

Educate consumers on medications, track logs, and conduct monthly audits.

Coordinate services based on individual needs.

Requirements

Possess a valid driver’s license & acceptable driving record.

Skills: Strong verbal and written communication skills, with proficiency in Microsoft Office Suite, including Outlook, Excel, and Word.
