Occupational Therapist, OT - Visits

About the Role

BAYADA Home Health Care seeks an experienced Occupational Therapist for per diem home health visits in Spartanburg, South Carolina. You will provide rehabilitative care to adult and geriatric patients in their homes across multiple counties.

Key Responsibilities

• Conduct home visits to evaluate patients' functional status including muscle function, endurance, and communication skills.
• Assess cognitive, psychological, and motor performance and develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Collaborate with nursing and multidisciplinary teams to revise care plans.
• Educate patients and families on rehabilitative care and safety.
• Document patient care using electronic medical records.

Requirements

• Current South Carolina Occupational Therapist license.
• Minimum one year recent OT experience.
• Graduation from accredited OT program.
• Ability to work independently with strong organizational and interpersonal skills.
• Computer proficiency; EMR experience preferred.
