About the Role

Join CentraCare Clinic as a Senior Transitions Physician – St. Cloud, MN in [object Object]. Join an expanding geriatric focused service line that provides mobile primary care services to multiple sub-acute, assisted living, and long-term care locations within a 60-mile radius of St. Cloud.

Key Responsibilities

will support our advanced practice providers in their onsite geriatric primary care practice for our long-term care patients by completing the physician required regulatory visits, rounding in our subacute settings, and help support our APPs with MD required services in our assisted living facilities.
