The Nurse Practitioner manages a defined patient panel, performing physical exams, ordering diagnostic tests, and formulating longitudinal care plans for patients with chronic conditions. They collaborate with interdisciplinary teams to ensure treatment alignment and provide care across multiple settings including homes, clinics, and via telehealth.
Candidates must hold a Master's degree in Nursing and have at least 2 years of experience as a Nurse Practitioner with a specialization in cardiology or nephrology. A current state license, national board certification, and a valid driver's license for regional travel are required.
