About the Role

Join Meals on Wheels of Southwest Ohio & Northern Kentucky as a Social Worker – M-F Schedule in United States.

Key Responsibilities

and responsibilities may include completing a thorough assessment of client situations to determine presence and degree of risk for abuse, neglect or exploitation for assigned client.

In collaboration with client and caretakers, if appropriate, develops a comprehensive care plan.

Implements and monitors (for reasonable time) care plan and delivery of services.

Establishes and maintains a positive working relationship with client and their caretaker (if applicable) encouraging them to utilize planned services to their optimal benefit.
