About the Role

Join Trumen Physicians and Associates as a Post-Acute NP – Flexible Schedule, Independent Contractor in United States.

Key Responsibilities

• Collaborate with Trumen physicians and facility medical directors to coordinate patient care

• Conduct comprehensive clinical assessments and manage treatment plans

• Support transitional care planning to help prevent avoidable hospital readmissions

• Reconcile medications and provide patient education consistent with scope of practice

Requirements

• Active Nurse Practitioner license in the state of Texas

• Minimum of 1 year experience as a Nurse Practitioner, preferably in a post

• acute, SNF, or geriatric care setting

• Ability to work independently while effectively collaborating with the care team
