• Conduct daily home visits using the agency vehicle.

• The Social Worker will create individual treatment plans based on the clients’ needs.

• Complete client assessments, relapse prevention plans, and housing applications.

Requirements

• paced environment. Valid and clean New York State Driver’s License required. PHYSICAL REQUIREMENTS This position requires sitting for long periods of time and travelling to conduct home visits.
