Key Responsibilities

• will see Home Health Patients.

• will assist patients and families to identify resources in the community.

• will work alongside a team of nurses, and therapist to help the patient regain prior level of functioning.

Requirements

• The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process.

• Per Diem position.

• Call responsibilities required.

• Day shift only - no nights.

• Flexible scheduling available.
