Key Responsibilities

will see Home Health Patients.

will assist patients and families to identify resources in the community.

will work alongside a team of nurses, and therapist to help the patient regain prior level of functioning.

Requirements

The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process.

17 SCHEDULE: Training 60

90 day Working 3 8hour or 5 8hour shifts for faster training (Days can vary from week to week depending on class schedule.) 8:00am

5:00pm Monday

Call responsibilities required.

Day shift only - no nights.
