Key Responsibilities
- Conducting assessments, diagnostic and functional evaluations of primary care patients
- Providing brief, focused intervention for patients in need of mental health services
- Providing consultation to primary care team providers to enhance skill and effectiveness in treating patients
- Providing timely feedback regarding care, treatment recommendations and progress of patients via documentation in patients’ charts and through verbal feedback
Requirements
- Bachelor’s degree plus a master’s degree from an accredited school of social work
- An appropriate degree and licensure by the Behavioral Sciences Board in Kansas (LSCSW, LCPC, or LCMFT).
- A commitment to following all legal and ethical guidelines and standards of the profession and of Heartland Community Health Center
- Excellent working knowledge of behavioral medicine and evidence
