About the Role

Join WellSpan Health as a Case Management Social Worker - York Case Management - Day in United States. Schedule Full Time, Day Shift (80 hours biweekly) Hours: Monday - Friday, 8:00am - 4:30pm Rotation: Occasional weekends General Summary Plans, organizes and implements social work for patients and families. Works in a team model to address patient care needs.

Requirements

A Bachelor's degree in social work or social welfare is required, along with less than 1 year of relevant experience. Basic Life Support certification is required upon hire, and a Licensed Social Worker credential is preferred.
