• Qualifications & Requirements: Licensure: Active Pharmacist License issued by the Illinois Department of Financial and Professional Regulation (IDFPR).

• Education: Bachelor’s Degree in Pharmacy (Required).

• Experience: Clinical pharmacy experience.

• Responsibilities Patient Care & Clinical Services Provide direct patient care by intervening as appropriate to improve prescribing, administration, and monitoring practices in the facility.
