About the Role
Join Huntsville Hospital Health System as a Social Worker – Days in United States.
Key Responsibilities
- Develops discharge plan based on the assessment and agreed upon goals with the patient, family and interdisciplinary healthcare team.
- Contributes to the interdisciplinary plan of care and ensures that the plan is consistent with the discharge plan and patients continuing care needs.
- Makes timely referrals to appropriate community services based on identified needs that assure continuity of care through the discharge planning process.
Requirements
- Maintains absolute adherence to hospital and departmental policies regarding confidentiality and patient’s rights.
- Perform additional duties as assigned.
- Qualifications Minimum Knowledge, Skills, Experience Required: Education: Require BS degree in Social Work from an accredited college or university and current Alabama licensure.
- Experience: Two years hospital experience preferred.
Work Schedule
- Full-Time position.
- Call responsibilities required.
