Key Responsibilities

• Develops discharge plan based on the assessment and agreed upon goals with the patient, family and interdisciplinary healthcare team.

• Contributes to the interdisciplinary plan of care and ensures that the plan is consistent with the discharge plan and patients continuing care needs.

• Makes timely referrals to appropriate community services based on identified needs that assure continuity of care through the discharge planning process.

Requirements

• Maintains absolute adherence to hospital and departmental policies regarding confidentiality and patient’s rights.

• Perform additional duties as assigned.

• Qualifications Minimum Knowledge, Skills, Experience Required: Education: Require BS degree in Social Work from an accredited college or university and current Alabama licensure.

• Experience: Two years hospital experience preferred.

Work Schedule

• Full-Time position.

• Call responsibilities required.
