Key Responsibilities

• Provide individual psychotherapy and case management for patients in both inpatient and outpatient settings

• Coordinate care across settings and connect patients to community resources. Documentation and Communication:

• Completes required documentation for record keeping and reimbursement according to the Service’s operating procedures.

• Maintain documentation and adhere to hospital policies and evidence

Requirements

• Ability to demonstrate authorization to work in the U. S. for any employer.

• Master’s degree from an approved school of social work (accredited by the Council of Social Work Education) is required.

• Must have active LCSW license.

• One year of social work in a health care setting is required.
