Key Responsibilities

• Conduct home visits within Monmouth County.
• Perform thorough evaluations of functional status including muscle function, endurance, coordination, communication, and ADL/IADL.
• Assess cognitive, psychological, and motor performance.
• Develop individualized intervention plans.
• Evaluate home environments for hazards and recommend adaptations.
• Collaborate with nursing staff on multidisciplinary care plans.
• Educate patients and families on rehabilitative care.
• Document care accurately using electronic medical records.

Requirements

• Current New Jersey Occupational Therapist license.
• Minimum one year recent OT experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently.
• Strong interpersonal and organizational skills.
• Computer skills; EMR experience preferred.
