Key Responsibilities
- Physical Therapists at DHHC screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
- The PT will: · Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
- · Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
- Communicate with all members of the health care team.
Requirements
- REQUIRED QUALIFICATIONS Must be a licensed Physical Therapist in Vermont and New Hampshire or hold a compact license.
- Driver’s license, minimum requirements on car insurance, and a clean driving record.
