Requirements

Candidates must hold a Doctor of Pharmacy (PharmD) or Bachelor’s Degree in Pharmacy (BS Pharm) and possess an active, unrestricted Tennessee Pharmacy license. Successful completion of a PGY1/PGY2 oncology residency or at least 3 years of clinical pharmacy/IDS experience is required.
