About the Role

Key Responsibilities

Conducts evaluations of prospective Gidwitz residents as part of the clinical team.

Conducts initial and ongoing assessments of the resident's emotional, physical, and functional status at Weinberg.

Assists new residents and their families transition into the community, as well as their transition out during discharge.

Helps educate families and residents on services available to residents living in Weinberg.

Requirements

Complies with safety policies and procedures.

Adheres to laws regarding the protection, use, disclosure, and release of client and CJE business information.

Attends required in

Reports violations of law, regulations, policies, or procedures.
