- Conducts evaluations of prospective Gidwitz residents as part of the clinical team.
- Conducts initial and ongoing assessments of the resident's emotional, physical, and functional status at Weinberg.
- Assists new residents and their families transition into the community, as well as their transition out during discharge.
- Helps educate families and residents on services available to residents living in Weinberg.
Requirements
- Complies with safety policies and procedures.
- Adheres to laws regarding the protection, use, disclosure, and release of client and CJE business information.
- Attends required in
- Reports violations of law, regulations, policies, or procedures.
