Summary/Objective Provides Medical direction and treatment of patients within the practitioner’s scope of practice and in collaboration with the supervising physician.

Essential Functions Exemplifies good customer service.

Collaborates with Multidisciplinary team in office and hospital to assure continuity of patient care.

Full-time position.

Hybrid position - combination of remote and on-site.

Requirements

Candidates must have a Master's degree from an accredited social worker program and possess a current Licensed Clinical Social Worker or Licensed Social Worker license in the State of Indiana. Preferred qualifications include two years of experience in a designated clinic, strong communication and critical thinking skills, and a current BLS certification or the ability to obtain one within one month of hire.
