Join Amana Living as a Social Worker – Full-Time, M-F Schedule in United States. The Role We currently have an exciting opportunity for a highly motivated, person orientated individual to join our organisation as a Social Worker. This is full-time, spanning across two of our facilities: Lefroy Care Centre (BullCreek) & Mosman Park Care Centre(Mosman Park), Monday - Friday.

Key Responsibilities

• Undertaking social work assessments, including psychosocial assessments for all assigned clients in consultation with other team members

• Liaising with clinical staff/ pharmacy and informing them of residents’ planned discharge dates for transport arrangements

• Case management may include supporting clients with State Administrative Tribunal (SAT) and guardianship processes.

• Planning and undertaking social work interventions with clients, in conjunction with their families and the multidisciplinary team
