About the Role

Join New England Life Care as a Clinical Pharmacist in United States. New England Life Care (NELC) is one of the fastest growing home infusion therapy services companies in New England and is the region’s only non-profit home infusion provider. NELC is a hospital collaborative serving more than 70 hospital systems in Maine, New Hampshire, and Massachusetts.

Key Responsibilities

Reviews prescriptions issued by physicians / authorized prescribers to assure accuracy and determine formulas and ingredients needed for dispensing the order.

Collaboratively assesses the suitability of each patient for home infusion therapy.

Assists the referral source in developing and implementing a plan for each patient’s smooth transition from hospital to home.

Develops, implements, and monitors patient’s individualized plan of care to include setting of therapeutic goals, monitoring of progress to goals and ongoing communication with prescribing physician.

Requirements

Demonstrates competency in the use and management of medication administration equipment.

Ensures that confidentiality of patient information is maintained in accordance with state and HIPAA Privacy and Security regulations. Educational / Professional Requirements:

Bachelor of Science degree from an accredited school of pharmacy.

Registered Pharmacist, in good standing, with a valid license. Skills:
