About the Role
Optimal Care seeks a Behavioral Health Care Manager to support mental and physical health care coordination for patients in a collaborative care team. This role focuses on managing common mental health and substance abuse disorders in outpatient settings.
Key Responsibilities
- Support mental and physical health care of patients.
- Screen and assess patients for mental health and substance abuse disorders.
- Facilitate patient engagement and follow-up care.
- Provide patient education on treatment options.
- Track treatment response and monitor clinical symptoms.
- Support psychotropic medication management.
- Provide brief behavioral interventions using evidence-based techniques.
- Participate in caseload consultation with psychiatric consultants.
- Document patient progress and treatment plans in EHR.
- Facilitate referrals and relapse prevention plans.
Requirements
- Bachelor’s degree in Social Work, Nursing, or Psychology.
- Minimum 2 years social work or care management experience.
- Current valid driver’s license and reliable transportation.
- Knowledge of mental health and substance abuse disorders.
- Experience with screening, assessment, and evidence-based counseling.
