About the Role

Optimal Care seeks a Behavioral Health Care Manager to support mental and physical health care coordination for patients in a collaborative care team. This role focuses on managing common mental health and substance abuse disorders in outpatient settings.

Key Responsibilities

  • Support mental and physical health care of patients.
  • Screen and assess patients for mental health and substance abuse disorders.
  • Facilitate patient engagement and follow-up care.
  • Provide patient education on treatment options.
  • Track treatment response and monitor clinical symptoms.
  • Support psychotropic medication management.
  • Provide brief behavioral interventions using evidence-based techniques.
  • Participate in caseload consultation with psychiatric consultants.
  • Document patient progress and treatment plans in EHR.
  • Facilitate referrals and relapse prevention plans.

Requirements

  • Bachelor’s degree in Social Work, Nursing, or Psychology.
  • Minimum 2 years social work or care management experience.
  • Current valid driver’s license and reliable transportation.
  • Knowledge of mental health and substance abuse disorders.
  • Experience with screening, assessment, and evidence-based counseling.

  • Salary range $55,000 - $62,000 annually.
