Partner with RN Complex Care Managers to support patients with complex needs

Conduct psychosocial assessments and identify barriers impacting care and safety

Support patients with cognitive challenges, dementia, and behavioral health needs

Assist with Medicaid applications and connection to community resources

Requirements

Bachelor’s degree in Social Work Required, Masters preferred

Wisconsin social work licensure required

3–5 years of social work experience required

Experience in case management, community
