Occupational Therapist, Home Health Visits

About the Role

This position is for an Occupational Therapist providing home health visits in Gaithersburg, Maryland. The role involves delivering rehabilitative care to clients in their homes as part of a multidisciplinary team.

Key Responsibilities

• Conduct home visits to clients within designated geographic areas.
• Evaluate patients’ functional status including muscle function, endurance, coordination, communication skills, and ADL/IADL performance.
• Assess cognitive, psychological, and motor performance and develop individualized intervention plans.
• Evaluate home environments for hazards and recommend equipment or adaptations.
• Continuously assess and revise care plans and collaborate with nursing staff.
• Educate patients and families on rehabilitative care and safety.
• Document observations and interventions accurately using electronic tools.

Requirements

• Occupational Therapy license valid in Maryland.
• Prior home care experience preferred but not required.
• Minimum of one year recent OT work experience.
• Graduation from an accredited occupational therapy program.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Computer proficiency; EMR experience preferred.
