Key Responsibilities

• In this role, you will provide supportive counseling to patients and families coping with complex medical diagnoses, treatment-related challenges, and psychosocial stressors.

• Conduct detailed psychosocial assessments and coordinate with community resources and supportive counseling.

• Manage all aspects of discharge planning for assigned patients in order to ensure a timely discharge and provide appropriate connection with post-discharge care providers.

• Educate patients, families and physicians regarding post-acute options and address freedom of choice and financial concerns.

Requirements

• A caregiver who excels in this role will: Coordinate the care and services of select patients.

• Intervene with patients who have complex psychosocial and discharge planning needs.

• Offer crisis interventions to patients and families with psychosocial needs and facilitate the transition of care for high-risk patients facing barriers to care.
