Home Health Field Registered Nurse (RN)

About the Role

Join 21st Century Home Health Services (21HHS) as a Home Health Field Registered Nurse (RN) in Petaluma, California. This full-time, in-field position involves visiting patients in their homes and providing skilled nursing care.

Key Responsibilities

• Provide hands-on patient care including PICC lines, wound care, feeding pumps, catheter management, trach care, blood draws, injections, IV antibiotic administration, and compression wraps.
• Implement care plans initiated by Case Management RN.
• Document care accurately within 24-48 hours.
• Educate patients and caregivers on diagnosis and care plans.
• Assist with skilled procedures and prepare equipment.
• Coordinate home health services with other disciplines.
• Participate in weekly case conferences.
• Maintain patient privacy and professionalism.
• Participate in on-call duties as required by weekend rotation.

Requirements

• RN license.
• One year of recent nursing experience preferred; may be waived for internal candidates or new grads.
