About the Role

Join 21st Century Home Health Services (21HHS) as a Home Health Field Registered Nurse (RN) in Petaluma, California. This full-time, in-field position involves visiting patients in their homes and providing skilled nursing care.

Key Responsibilities

Provide hands-on patient care including PICC lines, wound care, feeding pumps, catheter management, trach care, blood draws, injections, IV antibiotic administration, and compression wraps.
Implement care plans initiated by Case Management RN.
Document care accurately within 24-48 hours.
Educate patients and caregivers on diagnosis and care plans.
Assist with skilled procedures and prepare equipment.
Coordinate home health services with other disciplines.
Participate in weekly case conferences.
Maintain patient privacy and professionalism.
Participate in on-call duties as required by weekend rotation.

Requirements

RN license.
One year of recent nursing experience preferred; may be waived for internal candidates or new grads.
