About the Role

Join University Hospitals as a Clinical Pharmacist – Transitions of Care in [object Object]. We are seeking a proactive and collaborative Transitions of Care (TOC) Pharmacist to join our team. Reporting directly to the Retail Pharmacy, this specialized role is critical in bridging the gap between inpatient care and outpatient pharmacy services.

Key Responsibilities

Requirements

  • to safely and securely maintain Protected Health Information (PHI) for our patients.
  • Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
  • Education Bachelor's Degree in Pharmacy (Required) or Pharm D degree.
