Key Responsibilities
- & RESPONSIBILITIES Engages with newly assigned clients and their families by documenting contact within 2 business days of the referral.
- Conducts intake for clients entering services, ensuring understanding of all forms and obtaining necessary signatures.
- Develops individualized care plans that are person centered, family centered, and service centered.
- Assists clients in linking with internal and external resources as outlined in service plans.
