Key Responsibilities

• & RESPONSIBILITIES Engages with newly assigned clients and their families by documenting contact within 2 business days of the referral.

• Conducts intake for clients entering services, ensuring understanding of all forms and obtaining necessary signatures.

• Develops individualized care plans that are person centered, family centered, and service centered.

• Assists clients in linking with internal and external resources as outlined in service plans.

Requirements

• 20% Sitting

• 80% Squatting

• occasional Driving

• occasional Kneeling
