Key Responsibilities

• Integrate clinical guidelines, tools, and other metrics in the development of transition plans that are patient centered, promoting quality and efficiency in the delivery of care.

• Serve as a public awareness representativeand manage public and internal education relative to hospice services available and payor sources, especially the care connections program.

Requirements

• Qualifications Education and experience, essential Must be a graduate of an approved school of nursing, therapy, or social work.
