About the Role

Join Premier Health as a Physical Therapist – Home Care, Sign-On Bonus in United States.

Key Responsibilities

The Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences.

When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services.

The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.

The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.

3 years of job

directed and able to work with little supervision.  Computer skills preferred.  Home care experience preferred.
