About the Role

Key Responsibilities

• The Physical Therapist evaluates and then directly provides or supervises the provision of physical therapy services to patients in their homes or residences.

• When functioning in the capacity of the Home Care Case Manager (HCCM), the Physical Therapist is responsible for the provision and coordination of all components of a patient’s home care services.

• The Physical Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.

• The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.

Requirements

• 3 years of job

• directed and able to work with little supervision.  Computer skills preferred.  Home care experience preferred.
