About the Role

Join FreedomCare as a Registered Nurse in Kansas City, Missouri. This per diem, field-based role involves home visits to assess and support patients needing personal care services. You will work directly with patients in their homes to ensure quality care and independence.

Key Responsibilities

• Conduct clinical evaluations in patients' homes to determine care needs.
• Perform monthly visits to monitor patient conditions and service quality.
• Oversee caregiver training and competency.
• Maintain accurate and timely documentation.
• Coordinate care with physicians, RNs, program directors, and community resources.

Requirements

• Licensed Registered Nurse by Missouri Board of Nursing.
• Minimum 2 years clinical experience; 1 year with elders or adults with disabilities preferred.
• Valid driver's license and insured vehicle.
• Experience with home health or community-based services.
• Strong communication and organizational skills.
