Key Responsibilities

The Nurse Navigator coordinates personalized patient care, manages clinical needs, and acts as a liaison between patients and the multidisciplinary care team. They also facilitate the resolution of care gaps, provide patient education, and assist in the development of clinical pathways to improve health outcomes.

Requirements

Candidates must hold a bachelor's degree in nursing or a related field and possess at least two years of registered nurse experience in a clinical or hospital setting. Additionally, one year of experience in nurse navigation in a specialty area and a valid Colorado Registered Nurse license are required.
